Keeping your details up to date

It is essential any change of address be notified to the practice as soon as possible. Failure to do so may result in NHS Lothian withdrawing your registration. If you are unclear as to whether or not your new address is in our practice area, please ask at reception next time you are in or email our clinical email address.

Please also note if you leave the UK for a period of over three months you must advise us of this. NHS regulations require us to remove you from our practice list when you are absent for this period and you will require to reregister on your return to the UK.

If you move outwith the practice area, we can no longer retain you on our list. You will require to register with a local GP covering your new address. Details are available via the NHS Inform website, whereby you can enter your postcode and it will highlight practices that are potentially covering your address although this is not always 100% accurate.

If you cannot get registered at your new address, then you should email NHS Lothian to assist you with this on [email protected].

If you remain within UHS area then please provide us with your change of address details using the form below, completing as outlined.

Important Information for Patients changing Address

If you are waiting for a hospital appointment to be sent out it is your responsibility to inform the hospital of your new contact details. Please phone the hospital and ask for the relevant outpatient department.  Failure to do this will result in your appointment details being sent to your old address. In some cases you will  be removed from the department’s waiting list and require to be re-referred by your GP
