Keeping your details up to date

It is essential that any change of address be notified to the practice as soon as possible. Failure to do so may result in NHS Lothian withdrawing your registration. If you move outwith the practice area, you should register with a local general practitioner. When in doubt about your address being in the practice area please ask at reception. There are various ways to notify us of a change of address, telephone 0131 650 2777, email us at univhealthserv@nhslothian.scot.nhs.uk, use the pre-printed slip at the back of the patient booklet, call into the 3rd floor reception or write to us at Reception, Richard Verney Health Centre, 6 Bristo Square, Edinburgh EH8 9AL. (Please include your NAME and the names of any others in your household affected by the change of address, your CURRENT ADDRESS, your DATE OF BIRTH, your NEW ADDRESS and the date the change is effective from.)

Important Information for Patients changing Address

If you are waiting for a hospital appointment to be sent out it is your responsibility to inform the hospital of your new contact details. Please phone the hospital and ask for the relevant outpatient department.  Failure to do this will result in your appointment details being sent to your old address. In some cases you will  be removed from the department’s waiting list and require to be re-referred by your GP
