Change of Address
It is essential that any change of address be notified to the practice as soon as possible. Failure to do so may result in NHS Lothian withdrawing your registration. If you move outwith the practice area, you should register with a local general practitioner. When in doubt about your address being in the practice area please ask at reception. There are various ways to notify us of a change of address, telephone 0131 650 2777, email us at firstname.lastname@example.org, use the pre-printed slip at the back of the patient booklet, call into the 3rd floor reception or write to us at Reception, Richard Verney Health Centre, 6 Bristo Square, Edinburgh EH8 9AL. (Please include your NAME and the names of any others in your household affected by the change of address, your CURRENT ADDRESS, your DATE OF BIRTH, your NEW ADDRESS and the date the change is effective from.)
For confidentiality reasons please note that no medical correspondence or requests for appointments or prescriptions can be accepted through the Practice email address. If you are signed up for the online booking service you can book or cancel appointments or order repeat prescriptions using that service. (Online appointments)
If you have any suggestions whereby we might improve the service to you as a patient, please let us know. A suggestion box is available on the shelf at the entrance to the 3rd floor. If you would like a response to your suggestion then please let us have a return address.
